SPONSORSHIP AND SUPPORT
If you are requesting a sponsorship from Orthopaedic Associates of Wisconsin, please complete and submit the sponsorship form below. Submission of the form does not guarantee acceptance, as all requests are subject to review. Requests should be submitted at least 8 weeks prior to your event or sponsorship deadline to allow for proper consideration. The OAW Marketing Committee reviews sponsorship requests once per month, and you will be notified of the status of your request within 30 days.
