Tommy John Surgery (UCL Reconstruction)

Expert care for elbow injuries with advanced surgical options to restore strength, stability, and throwing performance.

What Is Tommy John Surgery?

Tommy John surgery, officially known as ulnar collateral ligament (UCL) reconstruction, is a procedure used to repair a torn or damaged UCL in the elbow. This ligament is crucial for stabilizing the elbow joint—especially in overhead athletes like baseball pitchers, tennis players, and javelin throwers.

What Causes a UCL Injury?

Common Causes

UCL injuries are most often caused by repetitive stress from throwing motions, which can lead to microtears or a complete rupture over time. Sudden trauma, such as a single hard throw or fall, may also cause damage.

Symptoms to Watch For

  • Inner elbow pain

  • Loss of throwing velocity or control

  • Swelling or bruising

  • Popping sensation at the time of injury

Who Needs Tommy John Surgery?

Athletes with full UCL tears or those who don’t improve with rest, therapy, or injections may benefit from surgery. It’s most common in competitive baseball players but also helpful for other athletes and active individuals with serious elbow instability.

What to Expect from Surgery and Recovery

The Procedure

The damaged UCL is replaced with a tendon graft from your own body or a donor. The procedure is typically done on an outpatient basis.

Recovery Timeline

Recovery can take 9 to 12 months, depending on your activity level. A structured rehab program is essential to ensure safe and complete healing, especially for athletes returning to throwing sports.
